Registration Information

Please take some time to read the following information before proceeding to register.

Registration can take up to 4 – 6 weeks to allow for transfer of records.

In order to be registered with the practice you must attend the Practice, with completed registration form, lifestyle questionnaire and photographic I.D., i.e. passport or driver’s licence and also a letter/bill as proof of address, where this is possible.

Cancelling Appointments – if you are unable to attend an appointment you must call 24hrs in advance, where possible.  If you fail to attend three consecutive appointments within 6 months, the practice will remove you from their patient list.

New Patient Prescriptions – you should obtain any repeat prescriptions from your previous practice before registering with us to allow us time required to obtain your medical records.

Medication Requests: We DO NOT routinely prescribe BDZs such as Diazepam, Lorazepam, Loprazolan, Lormetazepam, Nitrazepam, DHC,Temazepam, Co-codamol, Tramadol, Zopiclone, Zaleplon, Orlistat and Gabapentin.

We only prescribe GGC NHS Formulary, generic medications.

Letters and Forms – Please give 2 weeks notice for requests for letters and forms. Please note that there may be a charge for some type of letter, details of which are available in the surgery.

We do not provide housing support letters or sign passport forms.

Routine Appointments – If you are more than 10 minutes late for an appointment we must ask you to re-appoint.

NHS Zero Tolerance Policy  – Violence is defined as “any incident where staff are abused, threatened or assaulted.” This includes both verbal and physical abuse as well as threatening or abusive language.

Any incidents will be dealt with extremely seriously and may result in immediate removal from the surgery list or even prosecution.
