Care and Support at Home

Our aim to help people to remain at home for as long as it is safe and reasonable to do so. Where this isn't possible, we aim to ensure you are cared for in a safe and homely environment. We are involved in the assessment of adults aged over 16 who may have additional support needs due to physical or learning disabilities, long term conditions, old age or infirmity.
